Please fill out the following form for any new employees or employees in your department who are relocating. There will be some fields that do not apply to every situation. Submission of this form will send an e-mail to all of the appropriate people. Contacts are listed for each area if you have questions.

Furniture

(A minimum of 6 weeks notice is required if requesting new furniture.)

Facilities Modifications

Key Changes

(Campus Safety and Security – Associate Director #873-7869)


This request must be received in writing – email is OK – from the department director or chairperson who should provide (1) the name and Drury ID number of the person(s) authorized to receive the key(s) and (2) the building name and room number(s) involved. Please return existing key(s) to the Campus Safety and Security office.

Phone Service

(Technology Services – Help Desk Coordinator #873-7300)


(A minimum of 2 weeks notice is required to move a phone or request a new line.)

Computer Changes

(A minimum of 2 weeks notice is required if requesting a computer move and 4 weeks notice if requesting a new computer.)
